Care Management

Care management consists of a wide range of services and interventions that address every aspect of aging. 

A comprehensive assessment initiates the process and evaluates the client’s medical, social, emotional and financial well-being as well as their self-care abilities, safety needs and family relationships. 

The assessment determines the types of services and level of care that may be needed and matches the need to the appropriate resources through an individualized plan. Services may be as fundamental as overseeing household operations or as complex as arranging for the transition to residential placement.
